This form provides written notice of any changes that will be made to the student's Individualized Education Plan (IEP). This form is necessary during every IEP meeting and can be used for a variety of needs. Special educators may take some notes ahead of time, but the bulk of the form is completed during the meeting. This form may include changes in regard to the student's home situation, social history, psychological evaluations, or any other changes to their plan. The teachers must show data to support their reasoning for making any changes and must go through the form with parents to determine each part of the plan will be rejected or accepted. If rejected, a written explanation must be provided.
This form is used to determine a student's eligibility for certain accommodations based on his or her needs. This form describes the disability and specifies why particular services are needed. This form also provides reasoning and updates on educational reports, social history, and psychological evaluations if necessary. This form is filled in during an IEP meeting and must provide rationale for every accommodation.
These two form are used to analyze student behaviors when there is a behavior problem with the student. These forms are completed during IEP meetings and are used to show what the behavior problems are and how they will be addressed. Functional behavior assessment sheet 1A is filled out first. If behaviors worsen, 1B is completed. There are three parts to the 1B form: antecedent, behavior, consequence (ABC). It must also be determined whether or not the behavior is a result of the student's disability or personal choice. Using the information gathered and recorded, a final statement is made using the 1B information. There also needs to be proof that the behavior is affecting the student's academic performance.
After completion and analysis of a student's behavior using the Functional Behavior Assessment forms, a behavior intervention plan form may be necessary. This form is used to determine what actions may be necessary to help the student manage his or her behavior in the classroom. The form is completed before an IEP meeting and outlines the steps for disciplining the student when behaviors become a problem. The form also provides specific strategies that should be used to change and address behaviors.
